Answer: No it doesn’t come with leader or hook.

Dose this lure come rigged with hooks? If not, what size hook is recommended?

Answer: It comes rigged with a hook

Answer: No they do not come with hooks, they are normally used to skirt rigged ballyhoo, a long shanked 6/0 should work if you don't plan on trolling it with bait.

Answer: This is not rigged and is best used over a rigged bait such as a ballyhoo. If you are fishing for bigger game fish such as king mackeral or dolphin then I would suggest a 7.0 J hook.
